Wrestling Forms

These are the updated 2011 forms for USA Freestyle and Greco Wrestling.  You may use them for 2012.

For practice you are highly encouraged to buy a pair of wrestling shoes.  The takedown shop is a good store, as well as any other sporting goods store.  Knee pads and head gear are a great asset as well.  If you want to know why headgear is important, just look up cauliflower ear.  It's quite expensive to fix (as well as painfull and unsightly) so headgear saves money in the end. Sound iffy still?  Come watch a practice!!

New practice locations other than Johnson High school are up!  It is file SIX!!!  Go get some experience!

Parents or high school wrestlers, please print out, or at least take a look, at each of the files for your age group.  You can't get a USA card without the directions in the Parent handbook, so you need to review all information. 

A form that shows you how to access the website that lets you register for a USAW tournament is up.  It's almost like your own personal USAW webpage.  It takes about ten min, so when you have time, get it done.  You cannot register for a tournament without doing this. It is file SEVEN!!!

For grades K-5. (And really really tough four year olds :))

file1. USAW information sheet for K-5.

file2. USAW Parent Handbook. (Tells you how to get your USAW card)  It's ten pages, but the most important page is the one on how to get the USAW card.  We'll have extras at practice.

file3. *K-5 Registration for USAW.

file4. *Medical Release.

*Paperwork 3 and 4 must be filled out prior to first day of pracice, and you must have your usa card, no exceptions.

For middle school ages (6th-8th), regardless of school, print and fill out the following.

file4. **Medical Release.

For High School Signee's, (9th-11th)   Remember, if you are not a wrestler (kids who didn't finish the season this applies to YOU), you must fill out a new hall county athletic registration.  If you are not a football player or a wrestler, you must do the above AND go get an up to date physical or at least bring a copy of your up to date physical.  If you are not from JHS, print and fill out the following.
